rwtoweOur benefits pay $350. a month, for up to six months, following a hospital confinement of three or more days; or the benefits pay $72.50 per week, for up to 4 weeks, for convalescence following outpatient surgery or surgery requiring a hospital confinement of less than 3 days. A claim form must be completed which requires an Attending Physician’s statement directly on the claim form.
rwtowe IN-HOSPITAL COMFORT BENEFIT This benefit pays up to $25. a day for the cost
of non-medical, in-hospital “comfort” items such
as T. V. rental, telephone, hairdressing and reading material.. Receipts are not required when claiming the benefits.
